The Mental Health Crisis Program provides Health Care Navigation (HCN) services to assist clients in navigating the health insurance application process. Currently HCN’s are only able to receive referrals from the Crisis program.
HCN’s initiate outreach and assist with insurance coverage, General Assistance, Emergency Assistance, coordinate transportation and linking to other on-going services. HCN’s assist with accessing basic needs including: food, clothing, and medications.
