Resident Survey to Inform Development of Carver County Community Health Improvement Plan

The Carver County Public Health Department has begun work to develop a Community Health Improvement Plan for 2020-2024.

The County wants to better understand the health status of county residents as well as their health needs and concerns.  One way to build this understanding is through the completion of the 2018 Community Adult Health Survey.

Beginning this week, a randomly selected group of adults will receive the survey in the mail.  The voluntary survey and resulting information will not be identifiable with the resident’s identity, and the anonymous data will remain strictly confidential.   Carver County Public Health officials encourage residents to complete and return the survey.  It covers a variety of health issues, as well as circumstances that affect how people maintain their health or receive their healthcare services. 

“The information that we hope to acquire valuable in giving us a clearer picture of the current adult population’s health status” Dr. Richard Scott, Carver County Public Health Department Manager said.  “We are conducting the survey in partnership with Ridgeview Medical Center.  Both organizations want to know what is working well, as well as where there are areas for improvement, to help all of our residents to attain the best state of health”.

The survey is part of a larger data-gathering effort supporting Community Health Improvement Plan.  Conversations among community members took place in early October and will happen again in late November.  Public Health officials plan to combine survey data with other data to set the County’s public health goals for 2020-2024.

Scott noted that approximately 5,000 county households should receive the survey. Distribution splits evenly among the County’s eastern and western populations. The mailing is scheduled for delivery late next week, around Oct. 26.
