Finding Temporary and Long-Term Care Services in the Community
These days, there are many ways to receive services when you have temporary or long-term care needs. You may choose services that come to you, whether you live at home, at a seniors' residence or in other community settings. Community services may also help you move out of a long-term care facility.
Where can I find help to stay in my home?
- Call Carver County Senior Services at (952)442-7671 to talk to a person who can give you information about resources available in your community and help you set up a visit in your home, if you wish.
How do I know how much care I need or what services will help the most?
- Professional nurses and social workers are available for Long Term Care Consultation (LTCC) Services. The LTC Consultant can visit you and help you decide how much service you need, and what services are available in your community. This visit is free.
What happens during the Long Term Care Consultation (LTCC) visit?
- The social worker or public health nurse visits you within 10 working days of your request. Family members are welcome to attend if you wish. You'll talk about your general health, your ability to take care of routine daily activities, your home environment, and any help you may receive from family and friends. You'll also talk about your social needs and what type of supports and services are available in your community. Topics which may be discussed at a visit may include the following:
- Staying in your own home or apartment.
- Moving from a long-term care facility.
- Understanding your choices in services.
- Choosing services you prefer/need such as home care, equipment, assistance and support for caregivers, home delivered meals, homemaking, and more.
- Developing a plan to meet your needs.
- Accessing programs that help you pay for services if you are eligible.
- Ultimately the choice of what you do is yours.
How do I pay for services?
- Some people pay privately for services. At the Long Term Care Consultation visit we can discuss insurance or other payment sources which could help with the cost of services. Other people are eligible for assistance with part or all of the cost of services through state and federal-funded programs administered through Carver County, such as Alternative Care (AC) and Elderly Waiver (EW). If you are eligible for one of the programs through Carver County, our social workers or public health nurses can provide the care coordination once you are on the program.
Who can help me when I want to move out of a nursing facility?
- A Long Term Care Consultant can visit you and your family in a facility to help you plan services and other supports you need to return to the community. The visit is free. The consultant can also link people who participate in Medical Assistance to Relocation Services Coordinators to help them with the process of moving out of long term care facilities.
